DIRECTIONS TO FILL OUT ARD PAPERWORK

The following directions are for individuals who have been approved for ARD by the District Attorney’s Office.  In order to expedite the registration process after appearing before the assigned Judge, it is recommended that you complete the appropriate packet and to bring the documents with you on the date of your Court Hearing.  Please type or print neatly. 

Individuals applying for ARD for a non DUI offense will need to complete the following:

•     ARD Interview Card

•     Two (2) copies of letter regarding financial obligations

•     Two (2) copies of Rules and Regulations Governing ARD Probation

Individuals applying for ARD for a DUI Offense will need to complete the following forms:

•     ARD Interview Card

•     Two (2) copies of letter regarding financial obligations

•     Two (2) copies of Rules and Regulations Governing ARD Probation

•     DUI Administration form

      You must fill out the ARD Interview Card.  The following must be filled out entirely: Name, Address, Residing County, Home Phone #, Cell Phone #, Date of Birth (DOB), Social Security Number, Race, Sex (circle either Male or Female), U.S. Citizen (circle either yes or no); if no – include place of origin, and Employment (name, address, and phone number).  You are required to report by telephone (Call Track) once per month during your ARD probationary period. Cell phones are not permitted to make calls. Therefore, you must be sure to include a home phone or land line phone on this form. Please click here for more information on Call Track.

      You must sign and date both copies of the Financial Letter.  Sign only at “Signature of Probationer,” and “Date.” This letter explains your financial obligations.  Even if you pay in full the date of your ARD hearing,

you are still obligated to pay for Call Track, Safe Driving School, etc.; therefore this form must be signed. One copy will go to you to keep for your records and the other will go to Adult Probation.

You must read and initial the bottom of the first page of the Rules and Regulations Governing ARD Probation. Initial on the small blank line after “Client.”  On the second page, both copies are to be signed only at “Signature of Probationer,” and “Date.”  One copy will go to you to keep for your records and the other will go to Adult Probation.

If you are an ARD/DUI, the DUI ADMINISTRATION FORM needs to be filled out in addition to the above. The top part of the form needs to be filled out (NAME, DATE, ADDRESS, HOME PHONE AND WORK PHONE). 

You must circle either “yes” or “no” to indicate whether you had your CRN Evaluation completed already.  The CRN Evaluation is a list of questions given to determine whether treatment is needed or not. 

This would have been completed at the DUI Administration either in Willow Grove or King of Prussia.  The cost of the CRN is $50.  On the last part of this form, you need to circle where/when you prefer to complete your Safe Driving School (Alcohol Highway Safety School).  Your choices are 5 Tuesday nights

or 2 Saturdays (full days) in King of Prussia or 5 Wednesday nights or 2 Saturdays (full days) in Willow Grove. The cost of Safe Driving School is $200.  You will be notified through the mail by the DUI Administration as far as when your classes are scheduled.  If you cannot make the scheduled time, please contact the DUI Administration as soon as possible to reschedule.  The number is 610-992-7700 or 7728.  If you fail to appear as scheduled, you will be charged.
